Elbow fractures range from a small crack in the radial head to complex breaks needing surgery. The elbow is particularly prone to stiffness, so rehab usually starts as early as the fracture safely allows.

Common elbow fractures

Radial head fractures are frequent in adults after a fall on an outstretched hand. Olecranon fractures involve the bony point at the back of the elbow, often from a direct fall onto it. Supracondylar fractures, just above the elbow, are common in children after falls from swings or climbing frames.

Minor radial head fractures often need only a short period in a sling. Displaced fractures usually require surgery.

Why the elbow stiffens

The elbow has a tight joint capsule and closely fitting surfaces. Swelling, bleeding into the joint and immobilisation all lead to stiffness, especially losing the last few degrees of straightening. For many daily tasks, losing full straightening matters less than losing bending, since bending brings the hand to the mouth and face.

Rehabilitation

Early, gentle, active movement within the surgeon's guidelines is key. Aggressive forced stretching is avoided because it can increase inflammation and, rarely, contribute to abnormal bone formation around the joint. We use heat, gentle active and assisted exercise, and splints in some cases to gradually improve range.

Once movement improves, we build strength in the elbow, forearm and shoulder, and practise daily tasks. Recovery often takes three to six months, and some people are left with a small loss of full straightening that doesn't affect function much.

Loss of extension is common after elbow injury. Gradual exercise usually improves it.
Is forceful stretching helpful?
