The menisci are two C-shaped cartilage pads in the knee that absorb load and help the joint glide. Tears happen from twisting injuries in younger people and from gradual wear in older adults, and the two often need different approaches.

Two kinds of tear

Traumatic tears happen when the knee twists while bearing weight, such as turning sharply during football or badminton. They often come with other injuries like an ACL tear. Degenerative tears happen gradually in people over 40 as the meniscus loses resilience, sometimes after something as ordinary as getting up from a squat.

Degenerative tears are extremely common on MRI in pain-free people. Several high-quality studies have found that arthroscopic surgery for degenerative tears offers little advantage over physiotherapy for most people.

Symptoms

Pain on the inner or outer joint line, swelling that develops over hours, stiffness, clicking and sometimes catching. A knee that truly locks and can't be straightened suggests a displaced fragment and needs an orthopedic opinion quickly.

Rehabilitation

For most tears, we manage swelling, restore full knee straightening and bending, and build strength in the quadriceps, hamstrings and hips. Balance and control drills follow, then a graded return to running, twisting and sport.

After surgery

If a tear is repaired (stitched), rehab is slower and more protective, with limits on weight bearing and bending for several weeks so the repair can heal. If part of the meniscus is removed, recovery is usually faster, over a few weeks. Your surgeon's protocol guides us.
