Terminal Knee Extension

Terminal knee extension uses a band behind the knee to train the last few degrees of straightening while standing. It is used after ACL reconstruction and knee replacement to restore full extension and a normal walking pattern.

What it does

A knee that doesn't fully straighten makes walking tiring and loads the joint unevenly. TKE trains the quadriceps to lock the knee in standing.

You will often see it in our programmes for ACL tear, total knee replacement and meniscal injury, and it features in our knee pain physiotherapy programmes.

How to do it

  1. Loop a band around a sturdy object at knee height and step into it so it sits behind your knee.
  2. Step back until the band is tight and the knee is slightly bent.
  3. Straighten the knee fully against the band, squeezing the thigh.
  4. Hold 2 to 3 seconds, then slowly let the knee bend again.

How much

15 repetitions, 2 to 3 sets, once a day.

Common mistakes

  • Leaning the trunk back instead of straightening the knee.
  • Snapping the knee into hyperextension.

Stop and check with us if

  • The knee won't reach straight after several weeks of rehab.
