Tendon Gliding Exercises

Tendon gliding exercises take the fingers through five positions so each tendon slides fully within its sheath. They are used for carpal tunnel syndrome, trigger finger and after hand surgery to reduce stiffness and tendon adhesions.

What it does

The flexor tendons pass through the carpal tunnel. Gliding them fully may reduce swelling around them and improve movement.

Our physiotherapists use it most often for carpal tunnel syndrome, trigger finger and de Quervain's tenosynovitis, and it features in our joint pain physiotherapy programmes.

How to do it

  1. Start with the fingers straight.
  2. Hook fist: bend the top two finger joints, keeping the knuckles straight.
  3. Full fist: bend all joints.
  4. Table top: bend at the knuckles only, fingers straight.
  5. Straight fist: bend the knuckles and middle joints, fingertips touching the palm base.
  6. Return to straight between each. Hold each position 3 to 5 seconds.

How much

5 to 10 cycles, 3 to 5 times a day.

Common mistakes

  • Rushing through positions.
  • Forcing painful triggering fingers.

Stop and check with us if

  • Numbness is constant or thumb weakness develops in carpal tunnel syndrome.

Medical note

These instructions are general guidance, not a personal prescription. If you have a recent injury or surgery, a diagnosed condition, or symptoms such as numbness, weakness or dizziness, get assessed before starting. Stop and seek advice if pain increases sharply or you feel unwell.
