Scoliosis is a sideways curvature of the spine with a rotational twist. In teenagers the priority is monitoring and, where needed, bracing. In adults the focus is usually pain, stiffness and function.
Types and why they matter
Adolescent idiopathic scoliosis appears around the growth spurt with no known cause. It's more common in girls, and the main concern is whether the curve will progress while the child is still growing. Degenerative scoliosis develops in adults, usually after 50, as discs and facet joints wear unevenly. The two behave differently and are treated differently.
Curve size is measured by the Cobb angle on an X-ray. Small curves are often just monitored. Larger curves in a growing child are commonly braced. Very large or rapidly progressive curves may need surgery.
What physiotherapy can do for teenagers
Scoliosis-specific exercise approaches aim to teach the child to hold a more corrected posture using their own muscles, with breathing techniques directed into the concave side of the rib cage. There's reasonable evidence they help in mild curves and as a complement to bracing. They don't replace a brace when one is indicated.
We also look at practical things: school bag weight, sports participation (which should generally continue), and how the teenager feels about the brace. Adherence to wearing it matters enormously, and that often has more to do with confidence than information.
What physiotherapy can do for adults
Adult scoliosis pain usually comes from overloaded joints, muscle fatigue on one side and narrowing where nerves exit. We work on trunk endurance, hip mobility, balance and walking tolerance. Some adults get leg symptoms similar to spinal stenosis. Exercise doesn't straighten an adult spine, but it often reduces pain and helps people stay active.
