Flat feet are extremely common. Most flexible flat feet in children and adults cause no problems at all. The ones that do cause symptoms are often very manageable with strength work and the right footwear.

Flexible versus rigid

A flexible flat foot has an arch when sitting or standing on tiptoe, and flattens when standing. This is very common in children, who usually develop an arch by around age six to ten, and in many adults. A rigid flat foot stays flat in all positions and may need further investigation.

When flat feet cause problems

Some people with flat feet get arch fatigue, heel pain, shin pain or knee pain, particularly with long standing, walking or running. Flat feet that develop in adulthood, especially on one side, can be a sign of posterior tibial tendon dysfunction and should be assessed.

Children

Most children with flexible flat feet don't need treatment. Signs that warrant assessment include pain, stiffness, one-sided flatness, frequent tripping or difficulty keeping up with peers.

Treatment

For symptomatic flat feet, strengthening the foot muscles and the posterior tibial tendon, calf flexibility work and supportive footwear are key. Orthotics or insoles can help some people. Taping may provide short-term support. Hip strength is included where knee pain is involved.
