What happens in Sprengel's deformity

The shoulder blades form high in the neck region early in fetal development and then move down to their normal place. In Sprengel's deformity, one of them doesn't descend fully. It stays higher, is often smaller, and sometimes connects to the spine by a band of tissue or bone called an omovertebral bar.

Parents usually notice a raised or asymmetric shoulder, a short-looking neck on one side, or limited ability to raise the arm fully. It can occur alongside other conditions affecting the spine or ribs, so a medical assessment is the starting point.

When physiotherapy is the main treatment

Mild cases with reasonable arm function are often managed without surgery. Physiotherapy then focuses on keeping the shoulder blade as mobile as possible, strengthening the surrounding muscles, and helping the child reach and play without awkward compensations. Work is play-based for younger children; nobody gets far asking a four-year-old to do sets of ten.

When surgery is considered

If cosmetic concern or functional limitation is significant, surgical repositioning is sometimes done, usually in early childhood. Afterwards, physiotherapy helps recover range of motion and strength as the tissues heal. Parents are taught exercises to carry on at home, since frequent short practice matters more than occasional long sessions.
