Sacralization Physiotherapy in Indirapuram

Sacralization is a variation you're born with: the fifth lumbar vertebra is partly or fully joined to the sacrum. Many people never know they have it. For some, it changes how load moves through the lower back and hips.

What the variation looks like

At the junction between the lumbar spine and the pelvis, the last vertebra sometimes develops an enlarged side process that meets or fuses with the sacrum or pelvic bone. This is called a lumbosacral transitional vertebra. It can be on one side or both.

When it's linked to back pain, the pattern is sometimes called Bertolotti's syndrome. The theory is straightforward. If L5 can't move normally, the level above (L4 to L5) has to do more work and may wear earlier. A one-sided connection can also create an uneven false joint that becomes painful.

Symptoms that fit

Pain on one side of the low back, sometimes into the buttock, that's worse with side bending or twisting is a common complaint. People are often young adults, which is part of what distinguishes this from typical degenerative back pain.

It's worth saying clearly: finding sacralization on an X-ray doesn't prove it's your source of pain. We look at how symptoms behave with movement and testing before assuming it's the culprit.

How we manage it

We can't change the bone. We can reduce strain on the neighbouring segments. Hip mobility and strength, thoracic rotation, and trunk control spread movement more evenly. If one side of the pelvis is stiffer, targeted mobility work helps. Ultrasound and soft tissue techniques are used for irritated local tissues in the early phase.

If pain comes clearly from a one-sided false joint and persists despite good rehab, injection or other specialist treatment is sometimes considered. Your doctor can advise on that.

Exercises often used for Sacralization

These are exercises our physiotherapists commonly include. Which ones suit you, how many and in what order depends on your assessment and stage of recovery.

Medical note

This page is general information, not a diagnosis. Treatment is planned after an in-person assessment by a physiotherapist, and we refer you to a doctor where symptoms need medical review first. If you have sudden weakness, loss of bladder or bowel control, chest pain or severe unexplained pain, seek emergency care.

Frequently asked questions

Was I born with sacralization?
Yes. It's a developmental variation, not an injury or a degenerative change.
Do I need surgery?
Very rarely. Most people manage well with exercise and activity modification.
Why did pain start only now?
Symptoms often appear with a change in load, such as a new sport, heavier lifting or a period of inactivity.
