Lymphedema is persistent swelling, usually in an arm or leg, when the lymphatic system can't drain fluid properly. It commonly follows breast cancer treatment involving lymph node removal or radiation. It can't be cured, but it can be well controlled.

Causes

Secondary lymphedema follows damage to the lymphatic system from cancer surgery, radiotherapy, infection or injury. Primary lymphedema is less common and results from how the lymphatic system developed. In India, filarial infection is another cause.

Early signs

A feeling of heaviness or tightness, rings or clothing feeling tight, and swelling that initially improves overnight. Early treatment gives better long-term control.

Complete decongestive therapy

The standard approach combines several parts: manual lymphatic drainage, a gentle massage technique that encourages fluid toward working lymph nodes; compression with bandages or garments; exercise that uses muscle pumping to move fluid; and meticulous skin care to prevent infection.

Intensive treatment reduces swelling, then a maintenance phase keeps it under control, mostly through compression garments and self-care.

Preventing infection

Cellulitis, a skin infection, can worsen lymphedema. Keep the skin moisturised, protect it from cuts and insect bites, and seek prompt medical care if the limb becomes red, hot and painful.
