AI in Physiotherapy: How Technology Is Changing Rehabilitation

Artificial intelligence has arrived in physiotherapy the way it arrives everywhere: loudly, with big promises attached. Some of it is useful already. Some of it is a slide deck looking for a product.

What AI actually does in rehab today

Most AI in physiotherapy falls into a few practical categories. Movement analysis software uses a camera to estimate joint angles and track how someone squats, walks or lifts an arm. Exercise apps count repetitions and flag obvious form errors. Clinic software helps with scheduling, reminders and summarising notes. Research tools look for patterns in large sets of outcome data.

None of these replace the physiotherapist. They extend what a physiotherapist can see or free up time that used to go on admin.

Where it helps

The clearest benefit is between sessions. Most rehabilitation happens at home, where nobody is watching. An app that reminds you to do your exercises, counts your repetitions and shows your physiotherapist whether you actually did them fills a real gap. Adherence to home exercise is a long-standing problem in rehab, and anything that improves it matters.

Objective measurement is the second benefit. Knee bend after surgery, shoulder range, walking speed: these can be tracked more consistently with technology than by eye, and small changes show up sooner. Our piece on AI-powered movement analysis looks at how accurate that measurement is.

Where the claims outrun reality

Camera-based systems struggle with loose clothing, poor lighting, awkward angles and movements that happen out of the camera's view. Accuracy varies a lot between products and between joints.

More importantly, recognising that a knee drifts inwards during a squat is not the same as knowing why. Is it hip weakness, ankle stiffness, pain avoidance or habit? That judgement comes from assessment, history and hands-on examination. An algorithm can flag the pattern. It can't yet reason through the person.

There's also a risk of false confidence. An app that says your form is "perfect" doesn't know about the disc problem your doctor mentioned.

What this means for patients

Treat AI tools as helpers, not decision-makers. They're useful for reminders, tracking and feedback on simple movements. They're not a substitute for an assessment when something hurts or isn't improving.

If you use a rehab app, share its data with your physiotherapist. Numbers are only useful when someone qualified interprets them.

What we'd suggest asking before using an AI rehab tool

If you're considering an app or device that uses AI for rehabilitation, a few questions help separate useful tools from marketing. What exactly does it measure, and has that measurement been checked against clinical methods? Who designed the exercise content, and are they qualified? Can your physiotherapist see the data? What happens to your videos and health information?

It's also worth asking what the tool will do when something goes wrong. Will it flag increasing pain, or just keep counting repetitions? Tools that encourage you to contact a clinician when symptoms change are more responsible than those that don't.

Finally, consider whether it fits your recovery. For someone with a simple home programme, a basic reminder app may be enough. For complex rehabilitation, human guidance remains central.

Where it's heading

Expect better tracking, more personalised home programmes and tools that alert physiotherapists when a patient's progress stalls. Our overview of the future of physiotherapy covers what's coming. For now, the most effective rehab still combines a skilled clinician with a patient who does the work, and technology that helps both.
