Best Physiotherapy Exercises for Knee Arthritis

Exercise is one of the most effective treatments for knee osteoarthritis, often more effective for pain than people expect. The trick is choosing the right exercises and dosing them so they help rather than flare the knee.

Why exercise helps an arthritic knee

Arthritis changes the cartilage and bone, and those changes don't reverse. But pain in arthritis depends heavily on how well the muscles around the knee absorb load. Strong quadriceps reduce the forces going through the joint. Moving the knee regularly keeps it lubricated and less stiff.

That's why two people with similar X-rays can have very different amounts of pain.

The exercises that matter most

  1. Quadriceps sets. Tighten the thigh muscle with the leg straight and hold. Simple, but a good start when the knee is sore.
  2. Straight leg raise. Lift the straight leg while lying down. Strengthens without bending the knee.
  3. Long arc quads. Seated knee straightening, with an ankle weight once it's easy.
  4. Sit to stand. Standing up from a chair and sitting back down slowly. Functional and effective.
  5. Mini squats. Partial squats holding a support.
  6. Step ups. Stepping onto a low step, building towards stairs.
  7. Hip abduction. Side-lying leg lifts that strengthen the hip muscles controlling knee alignment.

Each has a step-by-step guide in our knee exercises section, including sit to stand.

How hard is hard enough

Mild discomfort during exercise is acceptable, around 3 to 4 out of 10 at most. What matters is how the knee responds afterwards. If pain settles within 24 hours and swelling doesn't increase, the load was fine and you can progress.

Two to three sessions a week of strengthening, plus daily movement such as walking or cycling, is a good target. Benefits typically build over 8 to 12 weeks.

What to go easy on

Deep squats and kneeling may be uncomfortable, but they aren't banned. It depends on the knee. High-impact activity such as jumping often aggravates symptoms early on. Swimming and cycling are gentle ways to add fitness.

Beyond exercise

Weight management makes a real difference in knee arthritis, because every step multiplies body weight through the joint. Supportive footwear and pacing activities through the day help too. Short-term treatments like heat, IFT or taping can ease pain while strength builds.

If you're wondering whether this approach can put off surgery, our article on delaying knee surgery covers that question directly.

A sample week

A practical programme for someone with moderate knee arthritis might look like this. On Monday, Wednesday and Friday, a 20 to 30 minute strength session: sit to stand, step ups, straight leg raises, mini squats and side-lying hip lifts, two to three sets each. On the other days, a 20 to 30 minute walk or cycle at a comfortable pace, plus a few minutes of gentle range of motion exercises.

Over the weeks, strength exercises get harder: more repetitions, a lower chair, a higher step, an ankle weight. Walks get gradually longer.

If the knee flares, reduce the load for a few days rather than stopping entirely, then build back up. Flares are normal and don't mean the programme is failing.

When to get help

If your knee is swelling repeatedly, locking or giving way, or pain is limiting walking despite regular exercise, an assessment helps adjust the plan. Arthritis rehab works best when it's progressive, and a physiotherapist can set the right starting point and move you forward safely.
