Why nights are worse

Lying on the painful shoulder compresses it directly. Lying on the other side lets the painful arm fall forward across the body, stretching irritated tissues. During the day, movement and distraction help; at night, there's nothing to take your mind off it, and the shoulder stays in one position for hours.

Common causes

Rotator cuff problems. Tendinopathy and tears commonly cause night pain, especially lying on the affected side.

Frozen shoulder. Night pain is typical in the early, painful stage, often severe.

Bursitis. Inflammation above the rotator cuff is aggravated by pressure.

Arthritis. Shoulder arthritis can cause night aching.

Referred pain from the neck. Neck problems can refer pain to the shoulder at night.

Sleeping positions that help

  • lie on your back with a pillow under the painful arm, so it rests slightly forward and away from your side
  • if you sleep on the good side, hug a pillow to support the painful arm
  • avoid lying on the painful side
  • a slightly propped-up position sometimes helps

Other tips

  • apply heat or ice before bed, whichever helps
  • take pain relief as advised
  • gentle pendulum exercises before bed can ease stiffness

When to get it checked

See a physiotherapist or doctor if night pain lasts more than two weeks, you can't lift the arm, the shoulder is stiff in all directions, or pain is severe. Night pain that's constant, unrelated to position, or comes with fever or weight loss needs medical assessment.

Frozen shoulder at night

Frozen shoulder deserves a special mention because its night pain can be severe. In the early, freezing stage, the shoulder capsule is inflamed and sensitive, and even small movements in sleep can cause sharp pain. People often describe waking several times a night.

During this stage, aggressive stretching usually makes things worse. Gentle movement within comfort, pain relief as advised by a doctor, and positioning the arm on pillows are more helpful. Some people benefit from medical treatments such as a steroid injection, which a doctor can discuss.

As the shoulder moves into the stiff stage, night pain usually eases even though movement is still limited. That's when more active stretching becomes useful. Matching treatment to the stage is the key, and it's why a clear diagnosis matters for anyone with persistent night pain and stiffness in all directions.

How physiotherapy helps

Treatment depends on the cause. Rotator cuff problems respond to progressive strengthening. Frozen shoulder needs stage-appropriate treatment, gentle in the painful stage and more active later. Our earlier article on frozen shoulder stages explains why timing matters.
