ACL Injury Recovery: How Physiotherapy Supports Rehabilitation

An ACL injury is usually a turning point. A pivot, a landing or a tackle, a pop, then swelling within hours. What follows is one of the longest rehabilitation journeys in sports medicine, and physiotherapy drives most of it.

Surgery or not?

Not every ACL tear needs reconstruction. People who don't play pivoting sports and whose knee feels stable after rehab sometimes manage well without it. Athletes in football, basketball, badminton and similar sports usually do better with reconstruction, because pivoting on a knee without an ACL tends to cause repeated giving way and further damage.

That decision is made with the surgeon, often after a period of physiotherapy to settle the knee and see how it responds.

Before surgery

Prehab helps. Reducing swelling, getting the knee fully straight and restoring quadriceps strength before surgery generally leads to a smoother recovery afterwards.

The phases after reconstruction

Early phase (0 to 2 weeks). Control swelling, achieve full knee straightening, switch the quadriceps back on with quad sets and straight leg raises, and walk with crutches.

Restoring movement (2 to 6 weeks). Full bend returns, walking normalises, and basic strengthening starts: mini squats, bridges, hamstring work as the graft allows.

Strength and control (6 weeks to 3 months). Progressive strength training, single-leg balance and control. The knee should feel normal for daily life.

Running and jumping (3 to 6 months). Once strength criteria are met, running begins, followed by jumping and landing drills.

Return to sport (9 to 12 months). Agility, cutting, sport-specific training, and then testing before full return.

Why it takes so long

The graft needs time to mature. Just as important, the knee's muscles and the nervous system's control of movement need time to rebuild. Returning to pivoting sport before about nine months is associated with a higher re-injury risk.

Testing before return

Return should be based on strength and function, not just time. Common tests compare strength and hop performance between the legs and check movement quality during landing and cutting. Our piece on return-to-sport rehabilitation explains what those involve.

The mental side of ACL recovery

ACL recovery is long, and the psychological side matters as much as the physical one. Many athletes feel frustrated by slow progress, anxious about re-injury and uncertain about returning to their sport.

Fear of re-injury is common and can affect performance and even increase injury risk. Gradual exposure to sport-specific movements, clear milestones and objective testing help build confidence.

Setting realistic expectations from the start helps too. Knowing that return to pivoting sport usually takes nine to twelve months, and that progress isn't linear, makes the journey easier to manage.
