People often expect a home physiotherapy session to be a lighter version of clinic treatment. A good one isn't. It's a full session, adapted to the space and focused on the tasks that matter at home.

The first visit

The first session is mostly assessment and usually runs longer than later visits. The physiotherapist will ask about the problem, medical history, medications, previous treatment and what daily activities are difficult. Reports and discharge summaries are useful to have ready.

Then comes the physical assessment: how you move, strength, joint range, balance, walking and specific tests for your condition. For older adults, this often includes timed tests like standing up from a chair and walking a short distance.

The physiotherapist also looks at the home itself. Bed height, bathroom layout, rugs, stairs and chair height all affect safety and independence.

Follow-up sessions

Later sessions follow a pattern:

  1. A quick check on how you've been since the last visit.
  2. Treatment: exercises, hands-on techniques, walking practice or electrotherapy with portable equipment where needed.
  3. Practice of specific tasks, such as getting out of bed, climbing stairs or using the bathroom safely.
  4. Updating your home exercise plan.

What a poor home session looks like

It helps to know what to watch for. A weak home physiotherapy service often shows the same signs: no proper assessment on the first visit, the same few passive exercises every session, no measurements, no written or demonstrated home plan, and sessions that end early.

Passive treatment, where the physiotherapist moves the patient's limbs while they lie still, has a place for people who can't move themselves. But for most patients, recovery comes from active work: the patient doing the movements, standing, walking and practising tasks, with the physiotherapist guiding and progressing it.

If sessions feel like they're going through the motions, ask questions. What are the goals? How are we measuring progress? What should we be doing between visits? A good physiotherapist welcomes those questions.

How progress is tracked

A good physiotherapist measures things: how many times you can stand up in 30 seconds, how far you can walk, how much knee bend you have. Those numbers show progress clearly.
