After ACL surgery
Physiotherapy After ACL Surgery in Indirapuram
ACL surgery physiotherapy in Indirapuram that begins with home visits in the first weeks, then moves to our clinic for the strength, balance and return-to-sport work a new graft needs.
Home visits first, then the clinic in Ahinsa Khand
Most people leave hospital after an ACL reconstruction on crutches, in a brace or a crepe bandage, with a knee that won't fully straighten and a thigh muscle that seems to have forgotten its job. For those first two or three weeks a car ride is uncomfortable and the walk into any building is a chore, so we usually start with home visits. Once getting in and out of a car stops being an event, sessions move to our Indirapuram clinic, because the later work needs equipment that won't fit in a living room.
That switch is easy for people who live close by. From Vaishali, Vasundhara or Kaushambi the drive is short, and someone can drop you on the way to work. If you're inside our Indirapuram home visit area you might do two weeks at home and the rest at the clinic. Someone further out may stay home-based for longer. Either way the same surgeon's protocol drives the plan.
Who this programme is for
This page is for anyone whose anterior cruciate ligament has been reconstructed: the footballer who landed badly, the office worker who twisted the knee on a wet staircase, the badminton player whose knee buckled on a lunge. It also fits people who had a meniscus repair or cartilage work done in the same operation, though those extras change the early limits. If the ligament was torn but not operated on, the ACL tear condition page explains the non-surgical route. For other knee ligaments such as the PCL or MCL, see our ligament repair rehab in Indirapuram.
Your graft shapes the first six weeks
Surgeons rebuild the ACL with a graft, most often taken from your own hamstring tendons, the middle strip of your patellar tendon, or part of the quadriceps tendon. Each choice leaves a different sore spot, and the rehab respects it.
- Hamstring graft: the back of the thigh is healing where the tendon was harvested, so loaded hamstring curls are held back and introduced gently when the protocol allows.
- Patellar tendon graft: the front of the knee can stay tender, and kneeling may be uncomfortable for months. We pad and pace kneeling tasks rather than banning them forever.
- Quadriceps tendon graft: the thigh is often slower to wake up, so extra early attention goes into getting it to contract properly.
A meniscus repair done at the same time usually means a limit on how far the knee may bend and how much weight it can take for the first several weeks, exactly as your surgeon specifies. We read the operative notes before the first session so none of this is guesswork.
ACL surgery physiotherapy in Indirapuram: the clinic phase
Somewhere between week four and week eight, depending on your surgeon and how the knee is behaving, the work changes character. Range of motion is mostly back, walking looks close to normal, and the job becomes rebuilding the strength the thigh lost while it was switched off. This is where the clinic earns its place.
Here we can load the leg in ways a home session can't match: gym equipment for controlled leg work, step platforms of different heights, balance boards, and enough floor space to practise changing direction. A mirror helps more than people expect. Watching your own knee drift inward on a single-leg squat teaches faster than any amount of being told.
Progression is earned, not scheduled. You move from two-legged squats to split squats to single-leg work when the previous level looks controlled and the knee stays quiet the next day. If it puffs up after a session, we back off and find out why. Our library of ACL rehab movements shows many of the exercises we draw on, but which ones you do, and how much, is decided after we've assessed you.
Recovery stages, roughly
The blocks below are a general outline. Your surgeon's protocol overrides it, and people move through it at different speeds.
Weeks 0 to 2 (mostly at home)
Control swelling, get the knee fully straight, wake up the quadriceps and walk safely with crutches. Muscle stimulation from our portable kit can help a thigh that refuses to contract.
Weeks 2 to 6
Bend past 90 degrees and keep going, come off crutches when the surgeon allows, rebuild a normal walking pattern, manage stairs. Many people shift to clinic sessions here.
Weeks 6 to 12
The strength block at the clinic: equipment-based leg work, step-downs, balance under load, cycling if permitted.
Months 3 to 6
Power and control. Jogging once the surgeon clears it, then landing practice and early agility drills.
Beyond 6 months
Sport-specific training and testing toward a return decision, made together with your surgeon.
Want a rough personal estimate? The ACL timeline tool gives one, as long as you treat its dates as a guide rather than a promise.
Return to sport is a test result, not a date on the calendar
The commonest mistake we see is going back to football or badminton because a certain number of months has passed. The graft goes through a weaker phase while it remodels, and the operated leg may still be clearly weaker than the other. Time is part of the decision. It isn't the whole decision.
Before suggesting a return, we look at side-to-side strength, hop and landing control, how the knee copes with cutting and pivoting, and how confident you feel (fear of re-injury is real and can be trained). The final word belongs to your surgeon. For the sport-specific end of rehab, our sports physiotherapy sessions run drills that match your game, whether that's cricket fielding or kabaddi.
When to call the surgeon or an ambulance
Contact your surgeon promptly for fever, a wound that turns red, hot or starts leaking, pain that suddenly becomes much worse, a knee that balloons after it had settled, or new numbness or weakness in the foot. Pain, swelling or tenderness in the calf can mean a clot and needs medical attention the same day. Sudden breathlessness or chest pain is an emergency: call 112 or 108 for an ambulance straight away.
What a clinic session looks like
Sessions are one-to-one with a qualified physiotherapist, and you can ask for a female physiotherapist. Expect a quick check of swelling and range, a main block of strength or control work, and a few minutes on what to do before the next visit. The clinic is open Monday to Saturday from 9 in the morning to 9 at night, and Sunday until noon, so evening sessions after work are realistic. We send progress notes to your surgeon so the next review has real numbers to look at.
If you live further out, the same rehab is organised differently. Our page on early ACL rehab at home across Noida covers the high-rise weeks after discharge, and ACL recovery in Raj Nagar Extension explains how we keep a home programme moving on a weekly plan. For how we handle other operations, see our post-surgical rehabilitation service.
Medical note
This page is general information, not a diagnosis or a treatment plan. Your physiotherapy is planned after an in-person assessment and works alongside the advice of your surgeon and doctor. Recovery differs from person to person, so no timeline or result on this page is a promise.
Frequently asked questions
Can I come straight to the clinic after discharge instead of having home visits?
Some people can, especially if a family member drives and the knee is already fairly calm. The test is simple: if the trip leaves the knee more swollen than when you set off, the session cost more than it gave. We can do the first assessment at home and decide together when clinic sessions make sense.
What is a hop test and when will I do one?
Hop tests compare how far, and how steadily, you can hop on the operated leg against the other one. They're done late in rehab, after the surgeon has cleared running and the knee no longer swells with exercise. They form one part of the return-to-sport decision alongside strength, movement quality and confidence, never the whole of it.
How many clinic sessions a week will I need?
Two or three supervised sessions a week is a typical starting point in the strength phase, tapering as you become reliable with the home programme. Some people train on their own between sessions once their technique is sound. The number is set after assessment and changes with your progress, your work timings and what the surgeon wants.
My knee clicks when I bend it after surgery. Should I worry?
Painless clicking without swelling is common after knee surgery and usually isn't a problem. Tell us and your surgeon if the click comes with pain, catching, locking, or swelling afterwards, because those can point to something such as a meniscus problem that needs checking before the rehab is pushed further.
Do you work with competitive players?
Yes. The principles are the same as for anyone else, but the final stage goes further: sport-specific drills, conditioning and formal testing before return. We coordinate with your surgeon and, if you have one, your coach, so training loads and rehab don't pull in different directions.
