After ankle surgery
Physiotherapy After Ankle Surgery in Indirapuram
Ankle surgery physiotherapy in Indirapuram after fracture fixation, ligament reconstruction, Achilles repair or arthroscopy, with home visits while you're in the boot and clinic sessions for the strength and balance phase.
Four ankle operations, four different starting points
Ankle surgery covers quite different operations, and the first weeks of rehab depend on which one you had.
- Fracture fixation (ORIF): plates and screws hold the broken bones, and weight-bearing stays limited until the surgeon sees healing on X-ray. The ankle fracture page covers the injury itself.
- Lateral ligament reconstruction: done for an ankle that keeps rolling over. The repair is protected from inward turning for some weeks.
- Achilles tendon repair: the foot is held pointing down in a boot with heel wedges, which come out in stages. Stretching the calf early is the one thing to avoid. More in the Achilles rupture guide.
- Arthroscopy: keyhole surgery to clear bone spurs or loose fragments. Usually a quicker start, though it depends on what was done inside the joint.
Read alongside your surgeon's notes, this tells us what you can and can't do in the first week.
Ankle surgery physiotherapy in Indirapuram: home first, clinic next
In the boot or cast phase, a trip to the clinic often isn't worth it. The foot needs to stay raised, crutches make the car awkward, and an ankle that swells up on the way home undoes the session. So early sessions are home visits within our Indirapuram home physio area, working on the hip and knee, toe movement, swelling and safe walking with the aid.
Once you're taking weight in the boot, or are out of it, the clinic becomes the better place. People coming from Vaishali, Vasundhara or Kaushambi can manage the short drive, and the later phase depends on equipment we have there.
Coming out of the boot
This is the stage people find hardest. The boot has been doing the ankle's job for weeks, and the first steps in a normal shoe feel unstable and strange. Expect swelling by evening and a limp you'll need to work on. Weight-bearing moves forward on your surgeon's instructions: often partial weight first, then full weight in the boot, then out of it. The weight-bearing calculator helps you understand what a given percentage of body weight actually means.
For an Achilles repair, the heel wedges come out one at a time, as the surgeon specifies, so the tendon is lengthened gradually rather than all at once.
The limp after ankle surgery has a recognisable shape. The foot turns out, the step on the good side gets short, and there's no push-off through the big toe because the ankle won't bend forward enough and the calf won't fire. Correcting it is slow, deliberate work: rolling through heel, midfoot and toes, matching step lengths, and walking at a steady pace rather than hurrying. A few minutes of that in front of the clinic mirror often shows more than a week of trying at home.
What clinic sessions add
The strength and balance phase is where ankle rehab is won or lost, and it's equipment-heavy. At the clinic we use balance boards and unstable surfaces, step platforms for calf raises off a ledge, gym equipment for loading the calf and thigh, and floor space for gait practice and later hopping. The wax bath, which we only have at the clinic, can loosen a stiff ankle before mobility work, once the wound has healed.
Stiffness is the other common problem. Some ankles regain movement quickly once the boot is off; others, particularly after a fracture with a long period in plaster, stay stiff when you try to bend the knee forward over the foot. That movement matters for stairs, squatting and getting up from the floor, so it gets regular attention, with hands-on joint mobilisation where it's appropriate and safe for the healing bone or repair.
We progress from balance on two legs to balance on one with eyes closed, from double-leg calf raises to single-leg ones, and eventually from walking to jogging and hopping where the surgeon allows. Balance board drills usually come in early in this phase.
The calf decides how well you walk
After almost any ankle operation the calf wastes quickly in the boot and comes back slowly. A weak calf shows up as a short stride on the operated side, trouble on stairs, and tiredness by late afternoon. After an Achilles repair it can lag behind for many months. We track it with a simple count of single-leg heel raises compared side to side, and we keep working on it long after walking looks normal.
Stages of ankle rehab
Protection (boot or cast)
Elevation, toe and knee movement, hip and core strength, safe transfers, walking with the prescribed aid and weight limit.
Early loading
Weight-bearing increases as allowed. Gentle ankle movement within the limits for your operation. Swelling control.
Out of the boot
Normal walking pattern, stairs, ankle range, early balance. Most people switch to clinic sessions around now.
Strength and balance
Calf and thigh loading, single-leg control, uneven surfaces.
Return to activity
Jogging, hopping, sport or work demands when the surgeon clears them.

When to call the surgeon or an ambulance
Contact the surgeon promptly about fever, redness, heat or discharge at the scar, pain that suddenly becomes severe, or toes that turn numb, pale or blue. A cast or boot that suddenly feels too tight needs checking. Calf pain or swelling above the ankle can signal a clot and should be seen the same day. Sudden breathlessness or chest pain: call 112 or 108 for an ambulance immediately.

Frequently asked questions
When can I wear normal shoes after ankle surgery?
When the surgeon takes you out of the boot. The first shoe should be supportive, with laces or straps you can loosen for evening swelling and a low, stable heel. Slip-on chappals are best avoided for a while, because the toes grip to keep them on and that changes the way you walk.
Why does my ankle swell more in the evening?
Fluid collects at the lowest point after a day on your feet, and an operated ankle is slower to clear it. Evening swelling that settles overnight is normal for months. Swelling that's still high in the morning, or that comes with calf pain or redness, needs checking by your surgeon.
Is the wax bath safe after ankle surgery?
Only once the wound has fully healed and the surgeon has no objection, and not over skin with poor sensation. It's used to make a stiff ankle easier to move before exercise, not as a treatment on its own. We check the skin and scar before using it.
Will the metal plate stop me playing sport?
Plates and screws don't usually limit sport once the bone has healed, though some people can feel the hardware under the skin or find it rubs in certain shoes. Whether it's removed later is the surgeon's decision. The rehab target is the same either way: strength, balance and confidence on the ankle.
